Self-Pay Practice
PureBliss Psychiatry currently operates as a self-pay (private pay) practice. We do not accept insurance plans directly.
This model allows us to:
- Provide personalized, unrushed care
- Maintain clinical independence
- Focus fully on your individual needs rather than insurance limitations
Out-of-Network Reimbursement
If you have out-of-network benefits, a superbill can be provided upon request. You may submit this to your insurance company for potential reimbursement, depending on your plan.
Please note:
- Reimbursement is not guaranteed
- Clients are responsible for verifying benefits with their insurance provider
Self-Pay Rates
Transparent Pricing
We believe in clear, upfront pricing with no surprises.
Psychiatric Evaluation (New Patient): $225
A 60–90 minute comprehensive evaluation including assessment, diagnosis, and treatment planning.
Follow-Up Appointment: $140
Includes medication management and supportive therapy as clinically appropriate.
Forms / Letters / Documentation Requests: $50
Includes brief work letters, school notes, and treatment summaries.
FMLA, Disability & Extended Forms
PureBliss Psychiatry completes FMLA, disability, and extended paperwork only for established patients who are actively receiving treatment.
Policy Requirements
- You must be an established patient (minimum treatment duration applies)
- New patients cannot request disability or FMLA forms at the initial visit
- An appointment may be required before forms are completed
- Extended paperwork carries a $75 fee, depending on complexity
These policies ensure accurate, ethical, and clinically appropriate documentation.
Accepted Payment Options
We accept the following payment methods:
- Credit cards
- Debit cards
- HSA / FSA cards
All payments are processed securely through our patient portal.
An active card on file is required for all clients.
Appointment Cancellations
- A 24-hour notice is required to cancel or reschedule
- Cancellations made within 24 hours or missed appointments will incur a $75 fee
- Fees must be paid before scheduling future appointments
